This cycle, CPCA and the Department of Health Care Services are advancing two complementary Medi-Cal navigation funding opportunities, each responding to a distinct set of federal eligibility changes now reshaping coverage for California’s most vulnerable residents. Though the two programs share a common application architecture and scoring methodology, they diverge in meaningful ways: the population each is built to serve, and the resources behind it.

$3.165 million total available, 2026 cycle
This program funds outreach, retention assistance, and access/utilization support for Medi-Cal members enrolled in or eligible for the Affordable Care Act Adult Expansion Group who are affected by H.R. 1’s new work and community engagement requirements, the shift to six-month renewals, and a shortened retroactive eligibility period.
Eligible health centers may apply alone, or lead a joint application with CHC and/or CBO subcontractors. Every applicant — lead and each subcontractor — must have previously participated in the AB 102 or SB 154 Navigator Projects.

This program funds outreach, retention assistance, and access/utilization support for Medi-Cal members with Unsatisfactory Immigration Status (UIS) who are transitioning from a managed care plan into fee-for-service Medi-Cal in January 2027.
Eligible health centers may apply alone or lead a joint application with CHC and/or CBO subcontractors. Regional consortia may not apply alone, but may lead a joint application — and may optionally deliver direct services themselves alongside their CHC/CBO subcontractors.

What Both Applications Share

| Joint applications need a tab-based Excel | One tab per organization delivering direct services in a spreadsheet. The lead entity needs its own direct-service tab when it delivers services. |
| Goals must stay proportional | Reported goals/outcomes must reflect only the work done for that program’s population of focus, sized to the award — not total organizational navigation volume. |
| Quarterly, deliverable-based payment | No advance or start-up payment. Reimbursement is issued quarterly based on progress across the three funded measures, plus timely reporting. |
