Embracing a Patient Centered Medical Home model where patient health outcomes are improving, care teams are working at the top of their licensure, practices increase their operational efficiency all while saving money is a difficult task to do alone. Successful Patient Centered Medical Home (PCMH) transformation is an organizational change where everyone is on-board with putting patients at the forefront of care. This program will prepare health centers in transitioning to a value-based care model and support them becoming a true health home.
About PCMH+
PCMH+ is a virtual training series offered by the California Primary Care Association with subject matter expertise provided by the Primary Care Development Corporation (PCDC) intended to support community health centers build capacity to apply for and achieve national PCMH recognition. The goal of PCMH+ is to provide an educational program available to health centers who would like to enhance their practice transformation capability to become a true patient centered health home and deliver high quality health care to patients when they need it, in a matter they can understand.

- 10 webinars
- 10 one-hour peer learning sessions, with facilitated technical assistance
- 5 hours of Practice Transformation Coaching
As part of this PCMH+ program, your organization will receive 5 coaching hours with CPCA’s Practice Transformation Coaches. You are able to utilize these coaching hours as needed for tailored technical assistance. Our coaches are experienced quality improvement professionals who can assist with documentation review, patient-provider empanelement, EHR reporting, care team redesign, workflow redesign, sharing best practices and much more. They can develop a custom coaching scope based on your needs and their vision will keep you focused on your practice transformation goals.
